# Form W-9 and Form 4669 collected by Stripe

A Form W-9 is generally used to confirm the tax identification number of a U.S. Person. Unlike a [Form W-8](https://support.stripe.com/questions/w-8-forms-collected-by-stripe), there is only one version of a W-9 and it’s applicable to both individuals and entities. The instructions that explain how to complete the form are attached to the form itself.
To start processing on Stripe, a Form W-9 isn’t initially required but a tax identification number is. Stripe may request a Form W-9 in order to verify your tax identification number (TIN) or if there’s a [change in information on your account](https://support.stripe.com/questions/change-in-circumstance-and-legal-documentation) since you last submitted a W-9.
### **Why am I being asked for additional documentation?**
If there are name and TIN discrepancies on your W-9 submission, we may need to collect additional documentation to complete verification. ### **Why can't I fill out the information for my single-member LLC?**
If your company is a single-member limited liability company (LLC) that has not elected to be treated as a corporation, the [IRS requires](https://www.irs.gov/businesses/small-businesses-self-employed/single-member-limited-liability-companies) you to enter the information of the single owner of the LLC.
### **Why are my payouts paused?**
Payouts may be paused if we are missing required information about your tax status or you have other outstanding requirements due for your account. Your account may continue to receive payments while payouts are paused, but you won’t be able to transfer the funds to your bank account, debit card, or any other external source.
To resume payouts, submit your W-9 tax form and complete any other outstanding requirements on your account.

Submission of a Form 4669 isn't required in all instances. Most Stripe account holders will only be required to complete the Form W-9 section.
## **Form 4669 collected by Stripe**
If you’ve submitted a [Form W-9](https://support.stripe.com/questions/w-9-forms-collected-by-stripe) to certify your US tax residency and Stripe hasn’t issued a [Form 1099-K](https://support.stripe.com/questions/1099-k-forms-issued-by-stripe) to you in previous years, there is a chance that Stripe will request that you review and sign a Form 4669.
A Form 4669 is a certification the account holder makes to confirm that the respective payment processing volume has been included in their previous tax returns, even though a 1099-K wasn’t issued for those years. Form 4669 essentially functions like an [Affidavit of Unchanged Status](https://support.stripe.com/questions/form-w8-and-affidavit-of-unchanged-status-collected-by-stripe) in that it is used to validate data from prior years.
